How Do I Refer a Patient?
To refer a patient to our practice, please call our office at (919) 256 2500 and inform our staff about the patient you are referring. We will then follow up with the patient and complete the referral process. Upon seeing the patient, your office will receive the results of the patients' visit.
The form below includes our contact information, directions, and other information that is relevant to your patient. We also offer pre-printed Referral Pads for any interested doctor, please contact our office to order a pad.
